Time is a valuable commodity that is in short supply and we understand how busy you are, therefore, we have designed this page to accommodate your schedule, including an easy way to begin your registration and enrollment process. All of the required information and documents have been linked here for your convenience. Simply print them, complete and bring them with you when you have made the decision to enroll with us.
In accordance with Maryland code 07.04.02.44 children will not be admitted or be allowed to remain in the Center without the required health inventory form (OCC 1215*) and completed emergency form (OCC 1214*).
All children need to have an up-to-date copy of immunization records to be enrolled in, and to attend school. If your child’s immunizations need to be updated and you can bring written proof of an appointment within 20 days to obtain the immunizations, you may temporarily enroll your child pending receipt of the required immunizations. Your doctor or health clinic can provide the DHMH 896 Form, or a computer-generated form, for you to take to the school. In addition, Maryland Child Care Regulations require that we provide you a copy of “A Parent’s Guide to Regulated Child Care“* and that we maintain a record verifying that you received it.
All parents enrolling their child(ren) into one of our programs will be required to complete an Official Registration Form*.
Fees, Tuition and Billing
Parents should review the Fees, Tuition and Billing Agreement*. Should questions arise, please contact the Center’s Director. Once reviewed and accepted, parents should sign the document and bring it with the rest of their enrollment package when the time comes for you to visit one of our Centers to register your child(ren).
